Insurance Information
Dr. Slabisak is contracted with most insurance plans including Medicare, Secure Horizons, Aetna POS, PPO and HMO, Blue Cross/Blue Shield PPO and HMO, United Healthcare, Cigna, Beechstreet, PHCS and Unicare PPO.
Appointments
You can schedule an appointment by calling 214-592-9955 during business hours. If you are unable to keep your scheduled appointment, kindly give us at least 24 hours notice so someone else may be seen. We often have many patients on a wait list.
Emergencies
If you are experiencing a medical emergency, call 911. If you feel you need to speak with Dr. Slabisak urgently regarding your medical problem, you can have the surgeon paged by calling 214-592-9955. Please listen to the entire message before making a selection. If you have concerns between appointments, please call our office and we will work together to ensure you receive the care you need.
Fees & Payments
We understand medical care can be expensive and we strive to provide the best possible service and care. Depending on your individual insurance plan, you may have deductibles, co-pays and/or co-insurance for services received. It is the patient’s responsibility for payment at time of service. For your convenience, we accept Visa, MasterCard, Discover, American Express, checks and cash. Note: If you have Medicare and do not have a secondary insurance, you will be responsible for your deductible and 20% co-insurance. We will take care of billing your insurance company and are happy to assist in handling your claims.
Medications/Refills
All medications, including refills, are prescribed based on your current condition. Follow-up appointments are necessary to monitor your condition and the effects of the medication prescribed. If it is time for a refill, please call your pharmacy and they will contact our office for approval. If you have not been seen in our office within 3 months, we cannot refill your prescription. Please make a follow up appointment so we can properly evaluate your condition and care plan. Please always tell our office if you have stopped or are taking any new medications when you visit. These policies help ensure you receive the best medical care. Pain medications will not be refilled after hours or on weekends.
Request for Medical Files
During the course of your care and afterwards, you always have access to your records and/or films. However, due to confidentiality regulations, we can only release your information to you or to your designee. We must have written, signed approval by the patient in order to release records to another physician, insurance company or even your spouse or family member. Please visit our office in person to sign a release form.
